Shock, fear, and confusion come rushing in. But even in fear, all parents need to know that leukaemia in children is now one of the most treatable and curable childhood cancers. Understanding how it works, the symptoms to watch for, and how the medical community treats it can provide immediate grounding and help turn a difficult diagnosis into a navigable care plan.

Leukaemia in Children

Leukaemia is a cancer of the blood and bone marrow. Every day, bone marrow produces immature blood cells called “stem cells”, which develop into three different types of blood cells:

Erythrocytes (red blood cells): They carry oxygen from the lungs to the rest of the body through the circulatory system.

Leukocytes (white blood cells): These cells fight viruses, bacteria, and infections.

Platelets (thrombocytes): These cells help form clots when blood vessels are damaged, preventing excessive blood loss.

In paediatric leukaemia, genetic mutations affect immature blood-forming cells and prevent them from developing into functional blood cells. Instead, these cells become abnormal, immature cells called blast cells. Unlike mature white blood cells, leukaemia cells can not perform normal immune defence functions.

More destructively, leukaemia cells crowd the bone marrow, displace healthy stem cells, and leave insufficient room to produce enough mature red blood cells, healthy platelets, and infection-fighting white blood cells. This loss of internal proportions is what accounts for the bodily symptoms of leukaemia.

Typical Symptoms

Leukaemia symptoms in children often tend to resemble those of common illnesses, such as the flu, or even growing pains. When several of the following symptoms occur together or worsen over time, prompt medical evaluation is important.

Signs of Anaemia (lack of RBCs)

Excessive lethargy: A once playful child may not want to do much and nap more. They fail to keep up with their friends during playtime.

Paleness: The skin, inner eyelids, gums, or nail beds may appear unusually pale.

Shortness of breath: A child may feel breathless or dizzy during even light physical activity because their body does not get enough oxygen.

Signs of Thrombocytopenia (low platelet count)

Unexplained bruising: Bruises may appear in unusual areas, such as the back, stomach, or face, without a known injury.

Petechiae: Tiny red-purple “dots” can appear when capillaries bleed. People often mistake this mark for an allergic reaction or a rash. They may appear all over the body.

Persistent nosebleeds or heavy gum bleeding: Frequent nosebleeds or gum bleeding, including during brushing, may occur.

Signs of Neutropenia (lack of WBCs)

Repeated fevers: Inexplicable or recurrent fevers that do not respond to usual remedies.

Chronic infections: Ear infections, throat infections, skin boils that just don’t go away and take time to heal even with medication.

Other Classic Signs

Bone and joint pain: The abnormal cells multiply and put pressure on the bones. Children may complain about leg, arm, or backaches. They might even develop a sudden limp.

Swollen lymph nodes: Lymph nodes of the neck, armpits and groin often swell. These painless lumps under the skin can be unusual signs of leukaemia in children.

Tight abdomen: Spleen and liver enlargement often causes this symptom. Leukaemic cells infiltrate these organs. A child might look bloated and report feeling full after only consuming a small amount of food.

Types of Childhood Leukaemia

The main types of childhood leukaemia that develop quickly may benefit from modern regenerative medicine.

Acute Lymphoblastic Leukaemia (ALL): Common in kids between 2-5 years. It occurs primarily from immature lymphocytes (B-cells or T-cells). ALL readily responds to modern chemotherapy regimens.

Acute Myeloid Leukaemia (AML): Requires highly intensive chemotherapy. AML occurs from normal progenitors for RBCs, platelets, and certain WBCs.

Mixed Phenotype Acute Leukaemia (MPAL): This rare disease is managed with carefully customised multi-agent therapies. It possesses characteristics of both lymphoid and myeloid lineage.

Chronic Myeloid Leukaemia (CML): Quite rare in childhood. It develops slowly and is managed with drugs taken orally.

Juvenile Myelomonocytic Leukaemia (JMML): This rare disorder in early childhood responds to stem cell transplant.

How is Childhood Leukaemia Diagnosed?

When a physician suspects leukaemia, they follow a systematic diagnostic pathway. These investigations determine the exact subtype and identify specific genetic markers, ensuring the medical team can design the most effective treatment plan.

Complete Blood Count: A routine blood draw reveals whether RBCs and platelets are depressed.

Bone Marrow Aspiration and Biopsy: A small liquid marrow sample and core fragment obtained from the child’s pelvis confirm acute leukaemia.

Flow Cytometry and Immunophenotyping: This laboratory analysis scans the surface proteins on the leukaemic cells. It identifies the origin of blast cells.

Cytogenetic and Molecular Testing: Advanced tests search for gene mutations. This helps categorise the cancer into specific risk tiers.

Lumbar Puncture (Spinal Tap): A sample of cerebrospinal fluid (CSF) collected from the lower back helps check for leukaemic cells in the spinal canal and brain. Doctors often administer small doses of preventive chemotherapy directly into the spinal fluid during this same procedure.

Stem Cell Transplantation in Managing Leukaemia

The approaches to childhood leukaemia treatment have evolved over the past decade. Haematopoietic Stem Cell Transplantation (HSCT) serves as one of the most powerful, potentially curative modalities for high-risk, refractory, or relapsed leukaemia. Rather than serving as an initial therapy for every patient, HSCT is reserved for cases where conventional chemotherapy alone is insufficient to prevent recurrence.

How It Works

Stem cell transplantation in leukaemia primarily relies on allogeneic donation. The curative mechanism operates on two fronts:

Intensive Conditioning: Before infusion, the patient undergoes high-dose chemotherapy, sometimes combined with Total Body Irradiation (TBI). This “conditioning regimen” destroys residual, resistant leukaemic cells and suppresses the patient’s immune system so it will not reject donor cells.

The Graft-versus-Leukaemia (GvL) Effect: The therapeutic heart of an allogeneic transplant is immunological. Donor stem cells engraft and produce a completely new immune system. Donor-derived immune cells recognise any remaining leukaemic blast cells as foreign and eliminate them.

Donor Selection

Successful transplantation depends on Human Leukocyte Antigen (HLA) matching to minimise immune incompatibility:

Matched Sibling Donor (MSD): Statistically the ideal match, but available to only about 25% of patients.

Haploidentical Donors: Half-matched family members (usually parents). Advanced techniques have made these transplants safe and widely accessible.

Umbilical Cord Blood: Highly tolerant of minor HLA mismatches, providing a fast alternative for patients requiring immediate transplantation without a matched donor.

Conclusion

A diagnosis of leukaemia in a child can be agonising for any parent. However, comprehensive treatment plans enable childhood blood cancer patients to reach full remission and enjoy productive, promising, and healthy futures.

Table of Contents Leukaemia in Children Typical Symptoms Signs of Anaemia (lack of RBCs) Signs of Thrombocytopenia (low platelet count) Signs of Neutropenia (lack of WBCs) Other Classic Signs Types of Childhood Leukaemia How is Childhood Leukaemia Diagnosed?
