Physiological edema or mild swelling in late pregnancy is the body’s response to a growing baby. During pregnancy, the mother’s blood volume increases to support the baby. Mild swelling is often a natural part of creating a new life. It helps to understand why it happens, how to find relief, and when inflammation can signal a serious medical issue.

Understanding Edema

Soon-to-be mothers may often notice swelling during the third trimester of pregnancy. This happens because blood and fluid volume in her body increases by almost 50 per cent during this time. This helps nourish the placenta and provide oxygen to the developing baby. This further makes the tissues more pliable for delivery.

All of that additional volume has to work against gravity and the body’s evolving structure:

The Expanding Uterus

As the uterus grows, it puts pressure on the pelvic floor and large blood vessels. One of these large veins, called the inferior vena cava on the right side of the spine, might get compressed and the blood that normally flows up the leg slows down, and the pressure of the blood rises in the legs, ankles, and feet. This causes clear, watery fluid to be pushed through tiny openings in the capillary walls and into the tissues, causing swollen feet during pregnancy.

Hormonal Shifts

High concentrations of progesterone signal muscle cells to relax throughout the body, including the walls of the veins. While this allows the heart to handle extra fluid without raising blood pressure, it can also make it harder for veins to propel blood back up to the heart. Meanwhile, aldosterone and estrogen enable the kidneys to hold on even more sodium and water.

External Factors

Being sedentary for extended periods of time, or consuming excessive salt from processed foods can lead to a quick gain in water weight.

Normal vs. Atypical Swelling

Signs of physiologic edema often follow predictable patterns:

Where: Usually confined to the fingers, feet, ankles and lower legs.

Symmetry: Swelling is usually similar in both feet and/or ankles.

Time of day: Swelling is usually negligible or absent in the morning and then builds throughout the day until evening.

Resting: Elevating the legs or lying on your side for a few hours may help.

Mild swelling during pregnancy is usually normal, but sudden or severe swelling requires medical attention.

Gentle and Effective Strategies for Relief

Managing third trimester swelling involves aiding venous return, aiding kidney filtration, and avoiding extra burden on the blood system:

Elevate and Rest Mindfully

One of the best ways to relieve lower-limb edema is to elevate the legs. Mothers need to prop their legs on pillows or footstools to keep them at or above hip level when sitting or resting.

Adopt the Left Lateral Sleeping Position

When lying down to sleep or rest, lie on the left side. Doing so takes the weight of the expanding uterus off the inferior vena cava, the largest vein in the body. This ensures uninterrupted blood flow to the heart and placenta.

Stay Hydrated

It may seem counterintuitive, but drinking enough fluids can help prevent excessive fluid retention. When the body registers mild dehydration, it produces hormones that promote retention of fluid and sodium. Drinking 2 to 2.5L of fluid a day helps flush out waste from the cells, regulate electrolytes, and help kidneys drain excess fluid.

Adding fresh lemon, cucumber or mint to water may encourage pregnant women to drink more.

Incorporate Gentle Movement

Gentle movement and exercise activate the calf muscles, which act as a secondary pump for the venous system. Twenty minutes of brisk walking 4 to 5 times a week helps prevent swelling during pregnancy.

When to Worry?

Swelling during late pregnancy is common; however, abrupt, rapid or asymmetrical swelling indicates an impending complication. The two most important signs to watch out for are Preeclampsia and Deep Vein Thrombosis (DVT).

Preeclampsia

Preeclampsia refers to a multisystem disorder that develops after the 20th week of pregnancy with elevated blood pressure and organ system stress. Capillaries may become more permeable due to dysfunction of the vascular endothelium. This results in fluid leaking rapidly into the tissues.

Critical Signs of Preeclampsia

Facial and Periorbital Edema: Rapid swelling and puffiness of the face, particularly around the eyes, before getting out of bed.

Sudden Weight Gain: An increase of around 2 Kg or more in a matter of days indicates rapid fluid accumulation and not a slow increase in the size of the fetus.

Non-Dependent Swelling: Significant swelling of the hands, wrists, or abdominal wall that does not improve following elevation or overnight rest.

Accompanying Red Flags

A severe or persistent headache that does not improve with medication.

Visual disturbances, such as blurred vision, flashing lights or floaters, or sensitivity to light.

Severe pain in the upper-right abdomen, just below the rib cage.

Unexpected nausea or vomiting in the late third trimester.

Deep Vein Thrombosis (DVT)

A DVT is a time sensitive condition that occurs when a blood clot forms in a deep vein, usually in the leg.
