For nine months, an almost silent, living ecosystem develops inside a mother’s uterus. Within this delicate system, the amniotic fluid foetal health grows and adapts to envelop, protect and nourish the new baby. Amniotic fluid can provide important clues about the baby’s well-being, kidney and lung function, overall health and much more.

Here’s all parents-to-be need to know about amniotic fluid and foetal health.

What Is Amniotic Fluid?

Amniotic fluid is a sterile liquid, typically clear to pale yellow, and warm. This liquid fills the amniotic sac where the developing embryo, and later the foetus, resides from the earliest days of pregnancy until delivery. Rather than being an inert pool, it is continuously produced, swallowed, absorbed by the baby, filtered through the baby’s kidneys, and then expelled again as fresh amniotic fluid. This is known as the “amniotic cycle.”

The early production of amniotic fluid during pregnancy occurs from several sources:

Initially formed from maternal blood, membrane secretions, and foetal skin, amniotic fluid transitions around 12 to 16 weeks of gestation to consist primarily of foetal urine and lung secretions. The baby continuously recycles this fluid by swallowing it, processing it through the digestive tract, and filtering it through the kidneys to produce more urine.

Functions of Amniotic Fluid

Amniotic fluid protects the foetus, supports its development, and promotes overall well-being.

Physical Protection and Temperature Regulation

One of the most important physical functions of the amniotic fluid foetal health is to cushion the baby against pressure, shocks, and impact to the expectant mother’s abdomen. It helps maintain a relatively stable temperature within the uterus, creating a suitable environment for the developing baby.

Assistance with Physical and Anatomical Growth

Amniotic fluid seamlessly helps the growing baby kick, roll, stretch and reach. Thus, helping train muscles for coordination and balance. It helps support optimal development of bone mass and musculature.

Assists with Lungs and Digestive System Development

Foetal development depends on the amniotic fluid for the baby to “practise” breathing movements. The baby moves amniotic fluid in and out of the developing lungs, supporting normal lung development.

The growing baby regularly swallows amniotic fluid. This helps support the development and function of swallowing and the digestive system, including the intestines. Amniotic fluid foetal health also protects the umbilical cord.

Measuring the Amniotic Fluid

Measuring amniotic fluid volume gives the healthcare provider immediate clues about placental function and whether the baby is urinating properly. Doctors use ultrasound during the second and third trimesters to check amniotic fluid levels and monitor the baby’s well-being.

If an ultrasound reveals unusually high or low amniotic fluid levels, doctors closely monitor the pregnancy.

Oligohydramnios means that the baby is surrounded by abnormally low amniotic fluid (generally calculated when the mother’s Amniotic Fluid Index (AFI) is under 5 cm or the Maximum Vertical Pocket (MVP) measures less than 2 cm). It occurs in about 4% – 8% of pregnancies.

The complications of low amniotic fluid include:

Premature rupture of membranes (PROM)

Placenta not functioning properly

Abnormalities in the baby’s kidneys or the urinary tract

Maternal complications

Drinking plenty of fluids can help prevent these situations.

Polyhydramnios is a situation where the baby is enveloped in extremely high amniotic fluid (AFI of > 24 cm or an MVP of > 8 cm). This develops in about 1%–2% of pregnancies, but is mostly mild.

Too much amniotic fluid causes:

Gestational diabetes

Difficulty with the baby swallowing, where the amniotic fluid can’t make its way through the GI system because of physical blockages in the gastrointestinal tract

Twin-to-Twin Transfusion Syndrome (TTTS), where identical twins share the same placenta, and one foetus might “steal” amniotic fluid from another.

In many mild cases, doctors may not find a clear cause, and the condition often resolves or remains stable without complication.

In many cases, mild polyhydramnios does not cause problems for the mother or baby. Depending on the cause and severity, treatment may include medication or, when necessary, removal of some of the excess fluid.

What Amniotic Fluid Tests Can Reveal

Beyond checking how much fluid surrounds the baby with an ultrasound, doctors can also test a small sample of the fluid itself to check the baby’s health. This test is called amniocentesis.

How is the test done?

The test is relatively simple. The doctor follows the baby with a live ultrasound and guides a thin needle through the mother’s abdominal wall into the amniotic sac. The doctor carefully removes about two tablespoons of the fluid and sends the sample to a laboratory for testing.

Why test the fluid?

As the baby grows, they naturally shed skin cells and other tiny particles into the amniotic fluid. Because this fluid holds the baby’s exact DNA and genetic signature, it gives doctors the following information:

Genetic Health & DNA

It allows doctors to look closely at the baby’s chromosomes and genes to check for conditions like Down syndrome or inherited genetic disorders (such as cystic fibrosis).

Lung Readiness

If an early delivery is imminent doctors need a baseline to assess amniotic fluid foetal health and readiness to breathe outside air by testing the fluid and finding out if the baby’s lungs have developed the necessary coating (Surfactant) to allow the baby to breathe easily on its own.

Checking for Infections

If the mother’s water breaks too early, testing the fluid helps doctors make sure no harmful bacteria or infection has entered the womb.

Amniocentesis is not a part of routine prenatal care, but still an elective option when specific issues may indicate an increased likelihood of a genetic abnormality, structural defect, or risk for illness during the pregnancy.

Conclusion

Amniotic fluid plays a vital role in creating a safe, nurturing space where your baby can grow and thrive throughout pregnancy.
