This disorder affects almost 5 to 8% of all pregnancies worldwide. Preeclampsia is the third leading cause of both maternal and perinatal morbidity and mortality. Detecting the <strong><em>early signs of preeclampsia <\/em><\/strong>during pregnancy and taking preventive measures is therefore imperative to manage the disease and avoid any untoward results. This article is an overview of factors that suggest risk, signs and symptoms to watch for, and the approach to diagnosis, all to facilitate early detection of preeclampsia.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" class=\"wp-block-heading\" id=\"what-is-preeclampsia\">What is Preeclampsia?<\/h2>\n\n\n\n<p>Preeclampsia is a life-threatening blood pressure disorder that often develops after the 20th week of pregnancy. Patients usually experience hypertension and proteinuria, where high levels of protein pass through their urine. Preeclampsia is characterised by a vascular abnormality affecting the liver, kidney, brain, and cardiovascular system. This condition is dangerous for both the mother and the developing foetus. When left untreated, Preeclampsia can develop into eclampsia or seizures, and HELLP syndrome, a life-threatening variant involving hemolysis, elevated liver enzymes, and low platelet count.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" class=\"wp-block-heading\" id=\"identification-of-risk-factors\">Identification of Risk Factors<\/h2>\n\n\n\n<p>Not all pregnancies are the same. It is standard practice in obstetrics that <a href=\"https:\/\/www.cordlifeindia.com\/blog\/effect-high-blood-pressure-pregnancy\/\">high blood pressure<\/a> be identified early in the first trimester. Maternal age of over 40 or under 18, with a medical history of chronic hypertension, preeclampsia, renal disease, Lupus (SLE), and type I and type II diabetes, indicates that the pregnancy is a high-risk one. Women over the age of 35 experiencing pregnancy for the first time and struggling with obesity run a lower risk of preeclampsia.<\/p>\n\n\n\n<p>High-Risk patients will often be candidates for prophylaxis, such as low-dose aspirin therapy, ideally started between 12 and 16 weeks of gestation.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" class=\"wp-block-heading\" id=\"clinical-warning-signs-and-symptoms\">Clinical Warning Signs and Symptoms<\/h2>\n\n\n\n<p>It is often difficult to detect <strong><em>preeclampsia warning signs<\/em><\/strong> in its early stages, as many of the signs and symptoms of the disease are identical to normal symptoms of a healthy pregnancy. Even so, any of the following signs and symptoms is a good reason for prompt evaluation:<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" class=\"wp-block-heading\" id=\"persistent-and-severe-hypertension\">Persistent and Severe Hypertension<\/h3>\n\n\n\n<p>Transient variations in blood pressure are normal. Persistent readings over 140\/90 mmHg on two occasions four or more hours apart define the clinical criteria of the disorder. Patients who are at high risk may benefit from home blood pressure monitoring to establish early elevated readings.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" class=\"wp-block-heading\" id=\"neurological-symptoms\">Neurological Symptoms<\/h3>\n\n\n\n<p>Vasospasm and cerebral oedema can result in neurological symptoms. Patients should be evaluated immediately for any of the following:<\/p>\n\n\n\n<ol>\n<li>Intractable Headaches: that are not relieved by standard pain relief medication.<\/li>\n<\/ol>\n\n\n\n<ul>\n<li>Visual Disturbances: blurred vision, photophobia or intolerance to light, and scotoma (seeing stars).<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\" class=\"wp-block-heading\" id=\"pain-in-the-epigastric-or-right-upper-quadrant\">Pain in the epigastric or Right Upper Quadrant<\/h3>\n\n\n\n<p>This is very commonly mistaken for heartburn or indigestion. Nevertheless, upper abdominal pain in relation to the right upper quadrant and under the right rib cage can be one of the <strong><em>signs of preeclampsia in pregnancy<\/em><\/strong>. This indicates inflammation or stretch on the liver capsule and calls for appropriate treatment.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" class=\"wp-block-heading\" id=\"sudden-onset-of-oedema\">Sudden Onset of Oedema<\/h3>\n\n\n\n<p>Swelling or oedema of the feet and ankles commonly occurs later in the third trimester due to venous compression. However, rapid facial, hand or foot swelling during pregnancy, preeclampsia, should prompt reporting to your healthcare provider. Pitting oedema in pregnancy that develops rapidly is alarming.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" class=\"wp-block-heading\" id=\"diagnostic-assessment-and-clinical-presentation\">Diagnostic Assessment and Clinical Presentation<\/h3>\n\n\n\n<p>Preeclampsia has previously had stringent <a href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/pre-eclampsia\">criteria<\/a>, but it is now known to develop without necessarily showing high protein levels in urine. Timely preeclampsia diagnosis tests can prompt medical intervention and avoid end-organ dysfunction.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" class=\"wp-block-heading\" id=\"laboratory-markers-of-preeclampsia\">Laboratory markers of preeclampsia<\/h3>\n\n\n\n<p>When clinical suspicion is high, healthcare providers utilise specific diagnostic tests:<\/p>\n\n\n\n<ol>\n<li>Urine Protein\/Creatinine Ratio: To ascertain protein levels.<ol><li>Serum Creatinine: To assess renal function.<\/li><\/ol><ol><li>Liver Function Tests (ALT\/AST): To monitor for elevated enzymes indicative of hepatic stress.<\/li><\/ol>\n<ol>\n<li>Platelet Count: To screen for thrombocytopenia.<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n\n\n\n<h3 class=\"wp-block-heading\" class=\"wp-block-heading\" id=\"home-bp-monitoring\">Home BP monitoring<\/h3>\n\n\n\n<p>Modern obstetric practice supports the provision of a home BP monitoring cuff. This enables the collection of longitudinal data. A rise in previously stable BP values, even below 140\/90, can signify developing preeclampsia and call for close monitoring to ensure maternal and foetal safety.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" class=\"wp-block-heading\" id=\"prevention-and-management-approaches\">Prevention and Management Approaches<\/h2>\n\n\n\n<p>While there is no way to eliminate the <a href=\"https:\/\/www.cordlifeindia.com\/blog\/pregnancy-with-pre-eclampsia\/\">risk of preeclampsia<\/a>, early diagnosis can prompt the initiation of management strategies that aim to extend the pregnancy into a safer gestational age.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" class=\"wp-block-heading\" id=\"low-dose-aspirin-therapy\">Low-Dose Aspirin Therapy<\/h3>\n\n\n\n<p>Low-dose (81 mg daily) aspirin is strongly recommended for women with obvious preeclampsia symptoms during pregnancy. Initiating this medication after 12 weeks&#8217; gestation can reduce the risk of early-onset preeclampsia.
